Scholarship Requirements

For Government-Funded Scholars:

  1. Notice of Award
  2. Certification of Grades from the Registrar
  3. Certificate of Good Moral Character from the Guidance Office
  4. Accomplished Application Form from the Scholarship Coordinator

For Privately-Funded Scholars:

  1. Certification from the sponsoring agency/association
  2. Accomplished Application Form from the Scholarship Coordinator

 

For Institutional Scholars: NLPSC-Employee Study Privilege and SG President.

  1. Certification from the Adviser (of the organization)
  2. Certification of Grades from the Registrar
  3. Certificate of Good Moral Character from the Guidance Office
  4. Accomplished Application Form from the Scholarship Coordinator

For NLPSC-Employee Study privilege:

  1. Accomplished Scholarship Form from the Personnel Office.
  2. Affidavit of Support
  3. Certification of Grades from the Registrar
  4. Certificate of Good Moral Character from the Guidance Office
  5. Accomplished Application Form from the Scholarship Coordinator

 

For Athletic Scholars:

  1. Accomplished Recommendation and Scholarship Form signed by the Coach, Adviser/Dean concerned, Sports Coordinator and Director for Academic Affairs.
  2. Certificate of Good Moral Character from the Guidance Office
  3. Accomplished Application Form from the Scholarship Coordinator

 

For Academic Scholars: College Scholar and Dean’s List.

  1. Certification of Grades from the Registrar
  2. Certificate of Good Moral Character from the Guidance Office
  3. Accomplished Application Form from the Scholarship Coordinator

 

For Entrance Scholars: Valedictorian and Salutatorian

  1. Certification from the School Principal that the applicant is a Valedictorian or Salutatorian
  2. Certificate of Good Moral Character from the Guidance
  3. Office/Principal.
  4. Photocopy of the High School Report Card (Form 138).
  5. Accomplished Application Form from the Scholarship Coordinator

 

For Barangay Scholars (BOS):

  1. Certification from the Office of the mayor that the applicant is a child of a barangay official.
  2. Oath of Office/Appointment of the Barangay Official.
  3. Photocopy of the Birth Certificate of the student-applicant.
  4. Certificate of Grades issued by the Registrar (for those presently enjoying the privilege) or High School Report Card/Form 138 (for those incoming freshmen)
  5. Income Tax Return (ITR) of the parents certified by the BIR
  6. Official.
  7. Certificate of Good Moral Character (from the principal or
  8. guidance counselor)
  9. Submission of an accomplishment report of the barangay official (first semester – First Quarter Report / second semester – Third Quarter Report)
  10. Accomplished Application Form from the Scholarship Coordinator

Provisions for qualification and renewal of scholarship: The coverage of the grant should only be within Ilocos Sur. In order to maintain the privilege, the grantee must have an average grade of 2.5.

 

Note: Filing, processing and approval of your scholarships and study grants shall be DURING THE ENROLMENT PERIOD until FIVE (5) DAYS AFTER THE CLOSE OF ENROLMENT.
