Admission Requirements

For New Entrants

• Original and photocopy of Form 138
• 2 copies of 2x2 recent picture in red background with name tag 
• Certificate of Good Moral Character 
• Original and photocopy of PSA Birth Certificate 
• PSA Marriage Certificate (if married women using husband’s family name) 

For Transferees 

• Official Transcript of Record or Certification of Grades
• 2 copies of 2x2 recent picture in red background with name tag 
• Honoral Dismissal/Permit to transfer 
• Certificate of Good Moral Character 
• Original and photocopy of PSA Birth Certificate 
• PSA Marriage Certificate (if married women using husband’s family name)

  For cross enrollees, students will present his/her permit to cross-enroll upon enrolment.
